While the technology promises to help dentists improve diagnosis accuracy, responsibility for treatment decisions and patient care still lies with the practitioner. By Shane Conroy<\/h2>\n\n\n\n<p>Artificial intelligence (AI) has landed firmly in Australian dentistry. One of the leading pioneers of the technology is US-based dental AI developer <a href=\"https:\/\/www.hellopearl.com\" target=\"_blank\" rel=\"noreferrer noopener\">Pearl<\/a>. Launched in 2019 by Ophir Tanz, founder and former CEO of contextual advertising firm GumGum, and Dr Kyle Stanley, a practising dentist and founder of Light Side Dentistry, Pearl has made a global splash with its <a href=\"https:\/\/www.hellopearl.com\/products\/second-opinion\" target=\"_blank\" rel=\"noreferrer noopener\">Second Opinion\u00ae<\/a> platform.\u00a0<\/p>\n\n\n\n<p>Second Opinion\u00ae received approval from the Therapeutic Goods Administration (TGA) in 2021 for use in Australia as an assistive tool in dental radiology, and is currently available to dental practices across the country.&nbsp;<\/p><div class=\"bitem-article-desktop-720x300-in-text\" id=\"bitem-3295918581\"><!-- Bite 720x300 [async] -->\r\n<script type=\"text\/javascript\">if (!window.AdButler){(function(){var s = document.createElement(\"script\"); s.async = true; s.type = \"text\/javascript\";s.src = 'https:\/\/servedby.engagemedia.com.au\/app.js';var n = document.getElementsByTagName(\"script\")[0]; n.parentNode.insertBefore(s, n);}());}<\/script>\r\n<script type=\"text\/javascript\">\r\nvar AdButler = AdButler || {}; AdButler.ads = AdButler.ads || [];\r\nvar abkw = window.abkw || '';\r\nvar plc705185 = window.plc705185 || 0;\r\ndocument.write('<'+'div id=\"placement_705185_'+plc705185+'\"><\\\/'+'div>');\r\nAdButler.ads.push({handler: function(opt){ AdButler.register(165450, 705185, [720,300], 'placement_705185_'+opt.place, opt); }, opt: { place: plc705185++, keywords: abkw, domain: 'servedby.engagemedia.com.au', click:'CLICK_MACRO_PLACEHOLDER' }});\r\n<\/script><\/div>\n\n\n\n<p>The software uses AI to assess patient radiologic images and deliver instant chairside detection of decay, infection, hard-to-spot issues like incipient caries, and the early signs of a periapical radiolucency, among other pathologies. It\u2019s built on Pearl\u2019s innovative computer vision technology.&nbsp;<\/p>\n\n\n\n<p>\u201cComputer vision is a subfield of AI that\u2019s specifically focused on teaching computers to see and interpret the world in much the same way that humans do,\u201d explains Pearl CEO and founder, Ophir Tanz. \u201cComputer vision means we\u2019re able to teach the machine things that are conceptual in nature versus what is explicitly programmed.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-the-upside-of-ai\">The upside of AI<\/h3>\n\n\n\n<p>Teaching the software to recognise a range of dental conditions requires a lot of training data. So the Pearl team built what they say is the world\u2019s largest collection of expertly annotated dental radiographs. \u201cWe feed millions of these annotated radiographs into the system to teach it how to detect all these pathologies,\u201d Tanz explains. \u201cIt\u2019s an evolving process. We take the daily radiograph data we receive globally into the system as training data, and deploy a new model every three weeks. So it\u2019s always improving.\u201d&nbsp;<\/p>\n\n\n\n<p>Second Opinion\u00ae also learns from its mistakes. Feedback from dentists using the software in daily practice is used to help update the system.\u00a0 \u201cWhen a dentist using the software identifies something they don\u2019t 100 percent agree with, it\u2019s registered in the system and goes through a panel of our clinicians,\u201d says Tanz. \u201cWhen they reach a consensus on a deficit or an area for improvement, it is fed back into the system.\u201d\u00a0<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>Computer vision is a subfield of AI that\u2019s specifically focused on teaching computers to see and interpret the world in much the same way that humans do. Computer vision means we\u2019re able to teach the machine things that are conceptual in nature versus what is explicitly programmed. <br><br><strong>Ophir Tanz, founder, Pearl<\/strong><\/p><\/blockquote><\/figure>\n\n\n\n<p>Pearl claims Second Opinion\u00ae has increased dentists\u2019 diagnostic accuracy by as much as 37 per cent, eases the diagnostic burden on dentists, and helps dentists to identify the early signs of some pathologies that are otherwise difficult to visually detect.<\/p>\n\n\n\n<p>\u201cSecond Opinion\u00ae can identify early indications that are often missed, like early bone loss, small carries, and interproximal carries or enamel reduction,\u201d he explains. \u201cThat makes Second Opinion\u00ae a good visual communication tool dentists can use to engage the patient in preventative treatment. That enhances the level of trust and is going to lead to more case acceptance.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-the-limitations-of-ai\">The limitations of AI<\/h3>\n\n\n\n<p>However, as Tanz notes, AI technology is not infallible, and neither is Second Opinion\u00ae. He says the software has been designed to serve as a second set of eyes for dentists. Ultimate responsibility for the accuracy of the diagnosis is with the practitioner.<\/p>\n\n\n\n<p>\u201cSecond Opinion\u00ae is a secondary reader; we don\u2019t replace the autonomy of the practitioner,\u201d he says. \u201cWe\u2019re trying to unburden dentists and enable them to be more accurate by using AI to point out areas of interest. They make the treatment decision.\u201d<\/p>\n\n\n\n<p>Data privacy and security issues associated with AI can be another sticking point for dentists considering bringing it to their practices. Tanz says it\u2019s an issue his company takes very seriously, and there are measures in place to maintain patient privacy.&nbsp;<\/p>\n\n\n\n<p>\u201cMost of our data in our lives at this point is in the cloud, but when you\u2019re dealing with medical data additional steps and protections are taken to de-identify and encrypt personal data,\u201d he explains. \u201cAll our data is heavily encrypted and de-identified. So even if we were to be hacked successfully, it would be impossible to associate patient names with the actual data itself.\u201d&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-the-future-is-now\">The future is now<\/h3>\n\n\n\n<p>Sydney-based <a href=\"https:\/\/www.gammatech.com.au\" target=\"_blank\" rel=\"noreferrer noopener\">Gamma Tech<\/a> is supporting the Australian roll out of Second Opinion\u00ae. Makenzie Harris, founder and director of Gamma Tech, says that while Australia is about three to four years behind the US in terms of AI, he\u2019s seeing strong interest from early adopters.\u00a0<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>I think the key thing for dentists to understand is that the radiology technology\u2014how we take X-rays\u2014hasn\u2019t changed. This is just the software catching up to help us get a better read on the image that\u2019s already in front of us. <br><br><strong>Makenzie Harris, founder, Gamma Tech<\/strong><\/p><\/blockquote><\/figure>\n\n\n\n<p>\u201cI think the key thing for dentists to understand is that the radiology technology\u2014how we take X-rays\u2014hasn\u2019t changed. This is just the software catching up to help us get a better read on the image that\u2019s already in front of us,\u201d he explains.&nbsp;<\/p>\n\n\n\n<p>\u201cThere\u2019s really not a steep learning curve involved. Our technical team can install the plugin remotely overnight, then when you arrive in your practice the next morning, Second Opinion\u00ae is ready to go. Then it\u2019s as simple as opening a web browser.\u201d<\/p>\n\n\n\n<p>Harris adds that there\u2019s an option to import several years\u2019 worth of historical data into Second Opinion\u00ae to provide a basis for comparison, and enable dentists to show patients a visual representation of deterioration or pathology progression over time.<\/p>\n\n\n\n<p>\u201cIt\u2019s all about detecting more disease so you\u2019re able to treat more preventatively, and being able to visually communicate that with the patient so they\u2019re more informed and engaged in their treatment,\u201d Tanz adds.&nbsp;<\/p>\n\n\n\n<p>\u201cIt\u2019s hard to imagine that in five to 10 years from now you\u2019ll walk into a practice that doesn\u2019t utilise it,\u201d Harris concludes.&nbsp; &nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Second Opinion\u00ae is bringing artificial intelligence directly into Australian dental practices.
