By Shane Conroy<\/h2>\n\n\n\n<p>Dr Tim Keys is on the frontline of Australia\u2019s paediatric dentistry crisis.<\/p>\n\n\n\n<p>As a practising dentist at the <a href=\"https:\/\/www.dentistryforkids.com.au\" target=\"_blank\" rel=\"noreferrer noopener\">Children\u2019s Dental Centre<\/a> on Queensland\u2019s Sunshine Coast and an <a href=\"https:\/\/aapd.org.au\" target=\"_blank\" rel=\"noreferrer noopener\">Australasian Academy of Paediatric Dentistry <\/a>(AAPD) representative, he\u2019s all too familiar with the systemic failings that are seriously compromising oral healthcare for Australian children.<\/p><div class=\"bitem-article-desktop-720x300-in-text\" id=\"bitem-2827153254\"><!-- Bite 720x300 [async] -->\r\n<script type=\"text\/javascript\">if (!window.AdButler){(function(){var s = document.createElement(\"script\"); s.async = true; s.type = \"text\/javascript\";s.src = 'https:\/\/servedby.engagemedia.com.au\/app.js';var n = document.getElementsByTagName(\"script\")[0]; n.parentNode.insertBefore(s, n);}());}<\/script>\r\n<script type=\"text\/javascript\">\r\nvar AdButler = AdButler || {}; AdButler.ads = AdButler.ads || [];\r\nvar abkw = window.abkw || '';\r\nvar plc705185 = window.plc705185 || 0;\r\ndocument.write('<'+'div id=\"placement_705185_'+plc705185+'\"><\\\/'+'div>');\r\nAdButler.ads.push({handler: function(opt){ AdButler.register(165450, 705185, [720,300], 'placement_705185_'+opt.place, opt); }, opt: { place: plc705185++, keywords: abkw, domain: 'servedby.engagemedia.com.au', click:'CLICK_MACRO_PLACEHOLDER' }});\r\n<\/script><\/div>\n\n\n\n<p>According to the <a href=\"https:\/\/ada.org.au\" target=\"_blank\" rel=\"noreferrer noopener\">Australian Dental Association<\/a> (ADA), thousands of children are being unnecessarily hospitalised every year for preventable oral health conditions. And when they require urgent treatment in hospital, many are unable to get access to the theatre.\u00a0<\/p>\n\n\n\n<p>The AAPD is estimating that several thousand children and special needs adults are being affected by this declining access.<\/p>\n\n\n\n<p>\u201cThat means many children and special needs adults are left suffering for long periods in pain and discomfort as a result,\u201d says Dr Keys. \u201cThe truth is that when it comes to paediatric and special needs dentistry, we have third-world service delivery in a first-world country.\u201d<\/p>\n\n\n\n<p>Dr Keys is seeing the real-world consequences of this on the ground. He\u2019s seen families that have been turned away from hospitals and forced to drive up to nine hours for private treatments.<\/p>\n\n\n\n<p>\u201cI recently saw a four-year-old patient with a large facial swelling who was turned away from their local regional hospital and told to go see the dentist,\u201d he says. \u201cThey then went to the public dentist, who was unable to provide any treatment as the child was too young, and the dentist could not get access to admit them for urgent surgery.&nbsp;<\/p>\n\n\n\n<p>\u201cSo the family contacted us in a very distressed state. The child had to be admitted into hospital on IV antibiotics until treatment could be provided. We ended up seeing them on the Monday morning, because there was a cancellation Sunday evening. This was just lucky for them. There are hundreds of similar stories.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-paediatric-dentistry-in-crisis\">Paediatric dentistry in crisis<\/h3>\n\n\n\n<p>Dr Keys was born into a family with a strong dental heritage. His father was a periodontist and his uncle was Queensland\u2019s first paediatric dentist. However, Dr Keys\u2019 pathway into the dental profession was a little different. After graduating from dental school at the University of Queensland in 2009, he joined the Royal Australian Air Force as a dental officer.&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>I recently saw a four-year-old patient with a large facial swelling who was turned away from their local regional hospital and told to go see the dentist. They then went to the public dentist, who was unable to provide any treatment as the child was too young. <br><br><strong>Dr Tim Keys, Children\u2019s Dental Centre<\/strong><\/p><\/blockquote><\/figure>\n\n\n\n<p>\u201cIt was a really good experience, particularly for a new graduate,\u201d he recalls. \u201cYou get to do high-quality dentistry and not worry about costs for patients. You also can provide the full scope of care to your patients.\u201d<\/p>\n\n\n\n<p>After his service, Dr Keys transitioned to private practice, where he initially worked as a general dentist. He honed his skills and developed a keen interest in paediatric dentistry, influenced by his undergraduate experiences in child oral health centres.&nbsp;<\/p>\n\n\n\n<p>He completed paediatric dentistry training at the University of Melbourne in 2018, and returned to Queensland where he took up his current role at the Children\u2019s Dental Centre on the Sunshine Coast.&nbsp;<\/p>\n\n\n\n<p>It didn\u2019t take long for Dr Keys to understand the shocking depth of the paediatric dentistry crisis. He points out that hospital funding for dental procedures is so inadequate that it does not allow for sufficient access for all oral health surgeries, both in the public and private sectors.&nbsp;<\/p>\n\n\n\n<p>The result is a significant reduction in the number of patients who can be treated, leading to multi-year waits in the public system.&nbsp;<\/p>\n\n\n\n<p>\u201cMany of these cases being left untreated for long periods are children and adults with additional medical needs such as autism or disabilities,\u201d Dr Keys adds. \u201cThe lack of access to necessary dental surgeries not only affects their physical health but also their emotional wellbeing. Chronic pain and repeated hospital visits can lead to severe anxiety and trauma, making future dental visits even more challenging.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-underfunded-and-untreated-nbsp\">Underfunded and untreated&nbsp;<\/h3>\n\n\n\n<p>Dr Keys explains that much of the problem lies in the Diagnosis Related Groups (DRGs) system. The DRGs system is used for funding, management, and planning in healthcare, and categorises hospital cases into groups expected to have similar hospital resource use.&nbsp;<\/p>\n\n\n\n<p>However, as it currently stands, the DRGs system does not accurately reflect the complexity and duration of paediatric dental procedures. This results in hospitals being underfunded for these treatments, which deters them from offering necessary surgeries.&nbsp;<\/p>\n\n\n\n<p>\u201cIn dentistry, there\u2019s one DRG for everything we do,\u201d Dr Keys explains. \u201cSo if we take one baby tooth out that takes five minutes, it comes under one DRG code. That means reimbursement rates are often insufficient to cover the actual costs of more complex procedures.&nbsp;<\/p>\n\n\n\n<p>\u201cPrivate health insurers will give about $900 to a hospital for a paediatric dental procedure. But it costs roughly $1500 an hour to run a hospital,\u201d says Dr Keys. \u201cThis discrepancy leads to hospitals losing money on dental surgeries, which prompts them to cut paediatric dental services.<\/p>\n\n\n\n<p>\u201cNearly every paediatric dentist in the country has had their lists cut or threatened to be cut. This has resulted in a reduction of patients that can be treated privately and a blowout to already multi-year waits in the public sector.<\/p>\n\n\n\n<p>\u201cThis is an ongoing medical emergency that has been largely overlooked by successive governments.\u201d&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-advocating-for-urgent-change\">Advocating for urgent change<\/h3>\n\n\n\n<p>This is obviously a dire situation that has serious implications for children in need of oral healthcare treatments around the country. But Dr Keys says relatively simple solutions can be applied to relieve the crisis.&nbsp;<\/p>\n\n\n\n<p>The AAPD is calling for urgent reforms including revising the DRGs system to accurately reflect the complexity and duration of paediatric dental procedures, ensuring adequate reimbursement rates from private health insurers, and increasing funding for public dental services.&nbsp;<\/p>\n\n\n\n<p>\u201cWe need the Independent Health and Aged Care Pricing Authority (IHACPA) to put more complexity into the DRGs system,\u201d Dr Keys asserts. \u201cAdditionally, private health insurers need to adequately reimburse private hospitals for dental procedures.\u201d<\/p>\n\n\n\n<p>Dr Keys also emphasises the lack of representation for dentists at both the federal and state government levels, which contributes to the ongoing neglect of dental health issues.&nbsp;<\/p>\n\n\n\n<p>\u201cThere is no chief dental officer in Australia, so dentists are reliant on other medical colleagues to advocate for dentistry,\u201d he explains. \u201cAnd despite the critical nature of these issues, the Federal Health Minister has not met with a single dental representative since taking office.
