Should you spill the beans or keep it to yourself? Angela Tufvesson investigates.<\/h4>\n<p>Every dentist is thrilled at the prospect of a new patient, especially given the potential for a long-term relationship and perhaps even recommendations to friends and family members. But developing a good relationship with a new patient isn\u2019t always straightforward. Unless the new patient is a young child, chances are they\u2019ve had dental work done in the past, which means you may need to contend with a range of techniques and degrees of professionalism. So what should dentists do if a patient complains about shoddy dental work or it\u2019s clear that previous work isn\u2019t up to scratch?<\/p>\n<h5>It\u2019s best not to\u2026<\/h5>\n<p>The simple answer is that dentists should avoid discussing the standard of work of other dentists with patients. The <a href=\"http:\/\/www.dentalboard.gov.au\/\" target=\"_blank\">Dental Board of Australia<\/a> (DBA) code of conduct says good relationships with colleagues and other practitioners strengthens the bond between dentist and patient, and enhances patient care. Specifically, it states that good practice involves \u201cacknowledging and respecting the contribution of all practitioners involved in the care of the patient or client, and behaving professionally and courteously to colleagues and other practitioners at all times.\u201d<\/p>\n<p>Likewise, the <a href=\"http:\/\/www.ada.org.au\/Home\" target=\"_blank\">Australian Dental Association<\/a> (ADA) code of ethics says dentists should act at all times in a manner that upholds and enhances<br \/>\nthe integrity, dignity and reputation of the profession.<\/p><div class=\"bitem-article-desktop-720x300-in-text\" id=\"bitem-2030659539\"><!-- Bite 720x300 [async] -->\r\n<script type=\"text\/javascript\">if (!window.AdButler){(function(){var s = document.createElement(\"script\"); s.async = true; s.type = \"text\/javascript\";s.src = 'https:\/\/servedby.engagemedia.com.au\/app.js';var n = document.getElementsByTagName(\"script\")[0]; n.parentNode.insertBefore(s, n);}());}<\/script>\r\n<script type=\"text\/javascript\">\r\nvar AdButler = AdButler || {}; AdButler.ads = AdButler.ads || [];\r\nvar abkw = window.abkw || '';\r\nvar plc705185 = window.plc705185 || 0;\r\ndocument.write('<'+'div id=\"placement_705185_'+plc705185+'\"><\\\/'+'div>');\r\nAdButler.ads.push({handler: function(opt){ AdButler.register(165450, 705185, [720,300], 'placement_705185_'+opt.place, opt); }, opt: { place: plc705185++, keywords: abkw, domain: 'servedby.engagemedia.com.au', click:'CLICK_MACRO_PLACEHOLDER' }});\r\n<\/script><\/div>\n<p>If a patient seeks advice from a dentist who isn\u2019t their usual practitioner about their oral condition, \u201cthe dentist should endeavour not to say anything which calls into question the integrity of their usual dentist\u201d.<\/p>\n<h5>And you can\u2019t really be sure<\/h5>\n<p>From a practical perspective, ADA president Dr Rick Olive says one of the main reasons to avoid bad-mouthing other dentists is it\u2019s impossible to know the exact circumstances under which treatment was provided to a patient. \u201cThere\u2019s an awful lot you can\u2019t tell by looking in the patient\u2019s mouth,\u201d he says. \u201cYou don\u2019t know necessarily when the treatment was done, and often dentists are asked to comment on things the patient said happened last week or last month or last year and what they\u2019re concerned about may have actually been done many years previously.<\/p>\n<p>\u201cPatients often have a very poor understanding of what\u2019s been happening in their mouth and of their previous treatments, so when a patient comes in and says, \u2018This was done by so-and-so two months ago and I think it\u2019s terrible\u2019, it\u2019s always sensible to be a bit cautious about taking that as the gospel truth.\u201d<\/p>\n<p>Dr Eryn Agnew, a community relations manager and professional consultant at the ADA Victorian Branch Dispute Assistance Service\u2014a free helpline that assists consumers with dental health and treatment related enquiries\u2014agrees that a patient\u2019s recollection of treatment can be fuzzy at best.<\/p>\n<p>\u201cIf you\u2019re not in the room you don\u2019t know what happened,\u201d she says. \u201cIt could\u2019ve been 5pm on a Friday, the patient is a terrible gagger and you can\u2019t treat them very well, they\u2019re going to a wedding the next day and just want a quick fix. So you do an emergency treatment but they don\u2019t come back for two years.<\/p>\n<p>\u201cSo what the new dentist may be seeing is a temporary job and the patient may not remember that or disclose it to the new dentist. The patient may have been given three or four different options and they may have selected a less-than-ideal solution, which has subsequently failed. Unless you know that entire history you shouldn\u2019t be commenting on why it happened.\u201d<\/p>\n<blockquote>\n<h5>\u201cWhat the new dentist may be seeing is a temporary job and the patient may not remember that or disclose it to the new dentist.\u201d \u2013<\/h5>\n<h5>Dr Eryn Agnew, ADA Victorian Branch Dispute Assistance Service.<\/h5>\n<\/blockquote>\n<h5>But you can\u2026<\/h5>\n<p>The DBA code of ethics requires practitioners to notify the board about other practitioners in some circumstances, including when they believe someone has \u201cplaced the public at risk of harm because the practitioner has practised the profession in a way that constitutes a significant departure from accepted professional standards.\u201d<\/p>\n<p>However, Dr Agnew says this practice should be reserved for extreme cases. \u201cSometimes dentists will tell patients to go to the <a href=\"https:\/\/www.ahpra.gov.au\/\" target=\"_blank\">Australian Health Practitioner Regulation Agency<\/a> (APHRA), but the regulator isn\u2019t really interested in single instances\u2014they\u2019re interested in a pattern of behaviour or outcomes that mean the public is at risk.\u201d<\/p>\n<p>But she says dentists do have a legal obligation to disclose any issues with treatment to patients. Importantly, this must be done objectively and without apportioning blame. \u201cIf you see something or do something yourself, you\u2019re meant to disclose it,\u201d says Dr Agnew. \u201cIf you see something that\u2019s not right, you need to say it professionally and objectively.\u201d<\/p>\n<p>Dr Philip Moss, a dentist at <a href=\"http:\/\/www.katoombadental.com.au\" target=\"_blank\">Katoomba Dental Centre<\/a>, says discretion is a must during consultations with patients. \u201cThis has happened to me many times,\u201d he says. \u201cI\u2019ve had to sit down and explain to the patient that more work needs to be done and discuss the work that has been done. The patient has sometimes been quite aghast about the fact that some previous work hasn\u2019t been up to scratch and they need to have more work done.<\/p>\n<p>\u201cMy response is not to accuse the previous dentist of not doing good work or bad work. I\u2019ve just been able to explain as I see it what needs to be done. I can\u2019t tell that patient that I feel their previous work has been sub-standard. It\u2019s about being very open about everything, but also about not making accusations about any other professional\u2019s abilities.\u201d<\/p>\n<p>Ultimately, Dr Olive says it\u2019s important to stick to just the facts, especially if a patient presses you for dirt on their previous dentist. \u201cIf the patient asks what the dentist thinks of their treatment, I don\u2019t think you have to cast any aspersions on another practitioner, but I think it is incumbent on the dentist to give an honest appraisal of the presenting condition.\u201d<\/p>\n","protected":false},"excerpt":{"rendered":"<p>You\u2019re examining a new patient and you realise the previous dentist did less-than-perfect work.
